Do You Need an MRI for Back Pain?

Not everyone with back pain needs an MRI. In fact, most cases of acute back pain improve within a few weeks with conservative treatment such as rest, activity modification, physical therapy, and anti-inflammatory medication. Ordering an MRI too early often doesn’t change the treatment plan and may even identify age-related changes that aren’t actually causing your symptoms.

So when is an MRI recommended?

Your doctor may order one if your symptoms suggest a more serious underlying condition or if your pain isn’t improving as expected. An MRI provides detailed images of the spine, discs, nerves, and surrounding soft tissues, making it one of the best tools for diagnosing conditions that aren’t visible on an X-ray.

An MRI may be appropriate if you have:

  • Back pain lasting longer than six weeks despite treatment
  • Pain that radiates into your arm or leg
  • Numbness, tingling, or muscle weakness
  • Suspected herniated disc or spinal stenosis
  • A history of cancer, infection, or significant trauma
  • Loss of bowel or bladder control, which requires immediate medical attention

It’s also common for a physician to recommend an MRI before considering procedures such as spinal injections or surgery. The scan helps pinpoint the exact source of the pain so treatment can be tailored to your specific condition.

That said, imaging is only one piece of the puzzle. MRI findings should always be interpreted alongside your symptoms and physical examination. Many people with no back pain at all have bulging discs or other spinal changes that appear on MRI but never cause problems. Treating the scan instead of the patient can lead to unnecessary procedures.
