New Patient Information
We are available to schedule appointments Monday through Friday from 8 – 5. Just give us a call at 855-774-6334. We kindly ask that you give at least 24 hours notice if you are unable to keep your scheduled appointment with us.
Prescription refills should be handled during business hours Monday through Thursday. We do not refill any prescriptions on Fridays. We require 72 hours notice when you need your prescription refilled. Please contact your pharmacy directly first so that they may request the refill for you.
No narcotics refills will be called in on nights or weekends.
We accept all major health insurance plans listed below. If you do not see your plan on the list, please contact our office as it may be possible that we are in the process of contracting with them.

- First Health/Coventry/Southern Health Services
- Humana/Choice Care

- Medicare and Medicare advantage plans
- Tricare (Prime)

- United Healthcare (all products)
- Virginia Health Network
All other insurances are eligible for participation, please check with our office.

Please remember to bring your insurance card and a valid driver’s license with you to your first visit. We look forward to meeting you!
We are happy to complete any short term disability paperwork or FMLA paperwork for a charge of $50 for each instance in which this service is needed.
